Research progress on the treatment of oral and maxillofacial hemangioma

Hemangioma is the most common vascular benign tumor in infants and young children, 60% of which occur in the oral maxillo-facial region. One characteristic of oral and maxillofacial hemangioma is spontaneous regression, which generally does not require treatment; however, a few hemangiomas can produce complications including ulceration, functional disorders and disfigurement, which require active treatments. Currently, the treatment of oral and maxillofacial hemangioma include drug treatment, laser treatment and surgical treatment. The drugs used to treat hemangioma mainly include beta blockers, glucocorticoids, alpha-interferon, imiquimod and antitumor drugs. Drug therapy is suitable for multiple, rapidly proliferating hemangiomas and hemangiomas that affect vital organ function or endanger life. Laser therapy can be applied to the early treatment of rapidly growing hemangiomas at exposed sites. Surgical treatment is suitable for proliferative hemangioma with serious complications, the reconstruction of any external deformity and the repair of a scar after an ulcer. Combined therapy and the development of new technologies provide new directions for the treatment of hemangioma but the efficacy remains to be proven by large sample prospective studies. Clinicians should appropriately evaluate the patients with hemangioma and develop individualized treatment programs for patients with treatment indications. This article reviews the efficacy, mechanism, clinical application and adverse reactions of different treatment methods and provides references for clinical treatment.

Study on the application of cone-beam CT in root canal morphology of first deciduous molars

Objective

To analyze the number, distribution of root canals in children's first deciduous teeth through a retrospective study of cone-beam computed tomography (CBCT).

Methods

A total of 185 first deciduous molars were selected from 91 children aged 4 to 8 years old in the Stomatology Department of Mianyang Hospital of Traditional Chinese Medicine. The number, distribution of root canals were analyzed.

Results

All 46 maxillary first deciduous molars had three roots; 139 mandibular first deciduous molars had two roots (77.70%) and three roots (22.30%). The root canals of all maxillary first deciduous molars are type Ⅰ, while the main root canal type of mandibular first deciduous molars is type Ⅳ. Of 51 children with bilateral mandibular first deciduous molars, 48 (94.12%) had symmetrical root and canal distributions.

Conclusion

The first deciduous molars usually have 2 to 3 roots. Root canals are mainly type Ⅰ and Ⅳ. The distribution of root canals is symmetrical.

Research progress on the influencing factors of dental fluorosis

Dental fluorosis is a kind of enamel hypoplasia caused by excessive fluorine intake during tooth development, leading to the formation of enamel with a lower mineral content and increased porosity, which can affect dental function and patients' appearance. The prevalence of dental fluorosis remains high on a global level, and its occurrence is affected by many factors. Excessive fluorine intake is a major risk factor for dental fluorosis. The ways of fluorine intake mainly includes the application of fluorides, daily diet and air. Since fluorides have been widely used in the prevention of caries in recent decades and the methods of exposure to fluorine have been increasing, increasing numbers of studies have been conducted to explore how fluoride can achieve a balance between the effective prevention of caries and the prevention of dental fluorosis. In addition, exposure to fluorine at earlier ages can also increase the risk of dental fluorosis, while the improvement of nutritional structure can affect dental fluorosis prevention. Genetic susceptibility, socioeconomic status and parental awareness may influence dental fluorosis. This article aimed to review the latest research progress on the factors influencing dental fluorosis and provide a reference for the prevention and treatment of dental fluorosis.

Research progress of probiotics in the prevention and treatment of oral infectious diseases

Oral infectious diseases include caries, periodontal disease, halitosis, candidiasis albicans and so on. Over the past few decades, probiotics have mainly been studied in the field of the gastrointestinal tract. In recent years, probiotics have begun to be used in the prevention and treatment of various oral diseases and have become a new field in the research of oral disease prevention and control technology. This paper reviews the research progress of probiotics applied in the prevention and treatment of various oral infectious diseases. A review of the literature shows that probiotics can prevent and cure dental caries by inhibiting the growth of Streptococcus mutans and competing with them for nutrition and attachment sites. Probiotics not only inhibit periodontal pathogens and reduce the production of sulfide, they also regulate the body′s immune function to alleviate halitosis and periodontal inflammation. Probiotics can inhibit periodontal caries by inhibiting them. Probiotics can inhibit them mycelial growth of Candida albicans and interfere with its adherence, thus playing a role in the prevention and treatment of oral candidiasis. Current studies have shown that pro-biotics play an auxiliary role in the treatment of caries, periodontitis, halitosis and oral candidiasis. However, the mechanism of probiotics in the prevention and treatment of oral infectious diseases is still unclear, and the safety of probiotics remains to be further studied. In the future, oral probiotics should be studied with reference to intestinal probiotics to better work to prevent and treat oral diseases.

Caries is an important public health problem that affect the quality of life of residents, and microorganisms is the most important factor in its incidence. Probiotics are types of microorganisms that are beneficial to the human body and can regulate the microecological balance; thus, they have great potential for caries prevention. This paper reviews the caries prevention mechanism of probiotics, the research progress of probiotics of different genera, and the early colonization of probiotics and their safety. The literature review showed that probiotics aimed at caries prevention could colonize the oral cavity and played a role in caries prevention by inhibiting the growth of cariogenic bacteria and regulating host immunity. At present, probiotics used to prevent dental caries include probiotic Lactobacillus, Bifidobacterium and Streptococcus. The positive effect of probiotics on dental caries has been confirmed in most clinical trials. Because probiotics include a variety of strains with strain-specific functions, different strains may be the reason for the lack of caries prevention in some clinical trials. The potential probiotic strains with a strong colonization ability and caries prevention effect in the oral cavity are the research hotspots for caries prevention probiotics.

Efficacy and safety of Bifidobacteria in preventing caries: a systematic review and meta-analysis

Objective

To systematically evaluate the efficacy and safety of Bifidobacteria in preventing caries.

Methods

Databases including PubMed, Embase, The Cochrane Library, Web of Science, Scopus, Clinicaltrials. gov, CNKI, WanFang Data and VIP were electronically searched from inception to April 2020 to collect randomized controlled trials of Bifidobacterium for caries. Meta-analysis was performed using Revman 5.4 software.

Results

In total, 10 randomized controlled trials (RCT) of 518 patients, including 262 in the test group and 256 in the control group, were included. Meta-analysis results reveal no statistically significant differences in salivary Streptococcus mutans counts (SMD=-0.31, 95%CI -0.66 to 0.04, P=0.08) (RR=0.53, 95%CI 0.17 to 1.66, P=0.28) and salivary Lactobacilli counts (SMD=-0.07, 95%CI -0.39 to 0.26, P=0.69) (RR=0.87, 95%CI 0.59 to 1.29, P=0.50). No statistical differences in the counts of Streptococcus mutans and Lactobacillus counts were noted in dental plaque, and no statistical difference in the occurrence of caries in deciduous teeth. Three of the 10 RCTS included in this study did not report adverse events, 5 had no adverse reactions, and 2 reported gastrointestinal discomfort.

Conclusion

Current evidence suggests that Bifidobacteria do not effectively reduce Streptococcus mutans counts and Lactobacillus counts in saliva and dental plaque, or reduce the occurrence of caries in deciduous teeth. The safety of this treatment also requires further investigation.

Research progress on biomarkers related to radiotherapy and/or chemotherapy - induced oral mucositis

Radiotherapy and/or chemotherapy-induced oral mucositis is a common oral complication in tumor patients undergoing radiotherapy and/or chemotherapy, which seriously compromises patients’quality of life and even affects anti-tumor treatment. Biomarkers are signal indicators that appear at different biological levels before or during disease. A comprehensive understanding of the biomarkers associated with oral mucositis contributes to the early identification of high-risk patients with oral mucositis and aids in the screening of patients prone to develop severe oral mucositis, guiding the prevention and treatment of oral mucositis. This article reviews the existing biomarkers associated with oral mucositis. The literature review results showed that the biomarkers associated with oral mucositis included growth factors, inflammatory cytokines, genes, plasma antioxidants, and pro - apoptotic proteins/inhibitor of apoptosis proteins. These biomarkers can be used to predict the risk of oral mucositis or facilitate early discrimination of patients prone to exhibit severe radiotherapy and/or chemotherapy-induced oral mucositis. EGF, TNF-α, IL-6, IL-1β and CRP can be used to predict and evaluate the risk and development of oral mucositis, whereas genes such as excision repair cross complementing 1(ERCC1), X-ray repair cross complementing 1(XRCC1), methylenetetrahydrofolate reductase (MTHFR) and tumor necrosis factor receptor superfamily member 1A (TNFRSF1A) have been focus of research in recent years. The genotypes and expression levels of some of these genes exhibit variable capacities to predict the risk and severity of oral mucositis. However, no biomarkers have been used in clinical practice, and more studies are needed in the future to verify the reliability and accuracy of these biomarkers, to provide a reference for the early accurate prevention and treatment of radiation and chemotherapy oral mucositis.
