Effect of auricular acupuncture combined with intermittent catheterization on urethral sphincter overactivity in the patients with neurogenic bladder after spinal cord injury

Objective

To observe the clinical efficacy of auricular acupuncture combined with intermittent catheterization on urethral sphincter overactivity in patients with neurogenic bladder (NB) after spinal cord injury (SCI).

Methods

A total of 90 patients with SCI were randomly divided into an auricular acupuncture group, an intermittent catheterization group and a combined treatment group, 30 cases in each group. All the patients in the three groups received the basic rehabilitation nursing and normal rehabilitation training. Additionally, in the auricular acupuncture group, auricular acupuncture was applied to “Spleen” (CO13), “Kidney” (CO10), “Bladder” (CO9) and “Subcortex” (AT4). In the intermittent catheterization group, the intermittent catheterization was operated. In the combined treatment group, the auricular acupuncture and the intermittent catheterization were delivered in combination. In the auricular acupuncture and the combined treatment group, the needles were retained for 1 h, once daily. One course of treatment was composed of 6 d and 4 courses were required. At the baseline and after interventions, the maximum urethral pressure (MUP), the residual urine volume (RU), the case number of urethral sphincter opening, the case number of vesicoureteral reflux, the number of white blood cells in urine, the case number of bladder function balance and traditional Chinese medicine (TCM) syndrome score were compared, with clinical efficacy evaluated.

Results

Compared with the baseline, MUP, RU, the case number of urethral sphincter opening, the case number of vesicoureteral reflux, the number of white blood cells in urine, the case number of bladder function balance, and TCM symptom score were improved in the combined treatment group after 4 weeks of treatment (P<0.05);and the therapeutic effects of the above indicators except the number of white blood cells in urine and TCM symptom score were improved more significantly when compared with the auricular acupuncture group and the intermittent catheterization group (P<0.05). In the auricular acupuncture group, MUP, RU, the case number of urethral sphincter opening, the case number of vesicoureteral reflux, the case number of bladder function balance and TCM symptom score were ameliorated in comparison with the baseline (P<0.05);while in the intermittent catheterization group, the improvements were obtained in the RU, the case number of urethral sphincter opening, the case number of vesicoureteral reflux, the case number of bladder function balance and the number of white blood cells in urine (P<0.05). The total effective rate in combined treatment group was significantly higher than that in auricular acupuncture group and intermittent catheterization group (P<0.05).

Conclusion

The combination of auricular acupuncture and intermittent catheterization is clearly effective on overactivity of the urethral sphincter in the patients with NB after SCI. It can significantly reduce the MUP, the RU, and the incidence of urinary tract infections in the patients, regulate bladder balance status, and attenuate the symptoms of TCM.

Progress of researches on acupuncture treatment of spinal cord injury by promoting axonal regeneration

Based on the growth factor theory, external inhibition theory, and neuronal intrinsic theory, we, in this paper, summed up the potential mechanisms of acupuncture in the treatment of spinal cord injury (SCI) by promoting the axonal regeneration in recent years. Acupuncture can promote the repairment of SCI by (1) regulating the expressions of spinal cord growth factors (such as nerve growth factor, brain derived neurotrophic factor, neurotrophin-3, glial cell line-derived neurotrophic factor, ciliary neurotrophic factor, insulin-like growth factor, etc.) to support neuronal survival and axon growth; (2) inhibiting the expression of spinal cord myelin growth inhibitor Nogo protein and oligodendrocyte myelin glycoprotein (OMgp), requlating axon guide molecules Sema3A and Wnt family, etc., and inhibiting the formation of astryocyte scar to create a favorable local microenvironment for axon regeneration; and (3) regulating spinal phosphatase and tensin homolog deleted on chromosome ten (PTEN)/mammalian target of rapamycin (mTOR) signaling, down-regulating Ras homolog gene family member A (RhoA)/ Rho-associated protein kinase Ⅱ (ROCK Ⅱ) signaling, up-regulating mitogen-activated protein kinase kinase (MAPK)/ extracellular signalregulated kinase (ERK) signaling, cAMP/ protein kinase A (PKA) signaling, etc.. In addition, we also found that by modulating axon regeneration, acupuncture therapy can not only effectively restore motor function in SCI patients, but also improve neurogenic bladder symptoms.

Research progress on the mechanisms of acupuncture in improving neurogenic bladder after spinal cord injury

The neurogenic bladder is a common manifestation of spinal cord injury. The occurrence and development of neurogenic bladder involves a variety of complex pathological mechanisms, including destruction of the structure and function of the detrusor muscles in the bladder tissue, abnormal excitatory conduction of bladder interstitial cells of Cajal, overactivation of the afferent C-fibers of the bladder, and loss of spinal cord neurons. It has been found that acupuncture is effective in the treatment of neurogenic bladder, and its underlying mechanisms are related to its functions in activating the bladder autophagy signaling pathway, modulating the expression of bladder fibroblasts molecules, regulating expression of proteins related to the contractile function of the detrusor muscles, stimulating excitatory conduction channel proteins on the Cajal interstitial cells of bladder, activating afferent C fibers within the bladder regulating spinal neuron regeneration, promoting the production of apoptotic factors in the spinal cord and bladder. In the present article, we summarized the research progress of the mechanism of neurogenic bladder after spinal cord injury and the mechanism of acupuncture and moxibustion interventions, so as to provide reference for elucidating the pathogenesis of neurogenic bladder and the role of acupuncture treatment.
