Effects of different concentrations of MTA on the proliferation and differentiation of stem cells from the apical papilla

Objective

To investigate the effect of different concentrations of MTA on the proliferation and differentiation of stem cells from the apical papilla (SCAP) and the potential of the SCAP to differentiate into odontoblasts.

Methods

SCAP were cultured in different concentrations of mineral trioxide aggregate(MTA). MTA experimental group with concentration of 0.01 mg/mL, 0.02 mg/mL, 0.1 mg/mL, 0.2 mg/mL, 1 mg/mL, 2 mg/mL, 10 mg/mL and 20 mg/mL were prepared. The number of cells at 1 day, 3 days, 5 days and 7 days were measured via a CCK-8 assay to observe the effect of MTA on SCAP proliferation. Real-time PCR was used to detect the gene expression changes. Cells cultured in alpha MEM culture containing 15% FBS without MTA were set as the control group.

Results

When cultured for 1 d, statistically significant differences in the promotion of in vitro proliferation of SCAP were not observed between each MTA experimental group and the control group (P>0.05). When cultured for 3 d, 5 d and 7 d, the 0.01 mg/mL MTA group presented obvious promotion of SCAP proliferation compared with the control group (P<0.05), whereas the 0.02 mg/mL, 0.1 mg/mL, 0.2 mg/mL, 1 mg/mL groups did not presented differences with the control group (P>0.05). The in vitro proliferation of the 2 mg/mL, 10 mg/mL and 20 mg/mL groups was lower than that of the control group (P<0.05). Real-time PCR detection showed that the expression levels of DSPP (t=-11.12, P < 0.05) and Runx2 (t=-10.62, P < 0.05) in the experimental group treated with 0.01 mg/mL MTA for 7 days were higher than those in the control group.

Conclusion

The 0.01 mg/mL concentration of MTA significantly promotes the proliferation of SCAP and shows the best ability to induce osteogenic and odontoblast differentiation in the SCAP, whereas high concentrations of MTA inhibited the proliferation of SCAP.

Distribution of oral Streptococcus mutans and its correlation with dental caries in children of Bortala Mongolian Autonomous Prefecture

Objective

To study the detection rate of Streptococcus mutans in oral cavities of 3-5- year- old Han, Uygur and Mongolian children in Bortala Mongolian Autonomous Prefecture, and the correlation between genotype and dental caries of preschool children.

Methods

Ninety children were randomly selected from the sample bank of children′s oral epidemiological survey data in the Bozhou area of Xinjiang. Forty-five children were included in the high caries group (more than 5 missing teeth), and 45 children were included in the noncaries group (0 missing teeth); each group comprised 15 children of each of the Han, Uygur and Mongolian nationalities. Plaque samples were collected and cultured with light saliva-bacillin agar medium and brain-heart infusion medium. Streptococcus mutans were cultured, and clinical isolates were further isolated and identified by Gram staining, biochemical identification and polymerase chain reaction. Genotype distribution was detected by random primer polymerase chain reaction.

Results

The detection rate of Streptococcus mutans in the 90 included children was 75.5%. The detection rate of Streptococcus mutans in the high caries group was 86.7%, which was significantly higher than that in the caries-free group (64.4%) (P=0.014). There was no significant difference in the distribution of Streptococcus mutans among Han, Uygur and Mongolian nationalities (P=0.457). A total of 549 clinical strains of Streptococcus mutans were obtained, and 113 different genotypes were found. In the high caries group, 61.5% carried more than one genotype of Streptococcus mutans, and 37.9% of the caries-free group had more than one genotype. The genetic polymorphism of Streptococcus mutans in the high caries group was significantly higher than that in the caries-free group (P=0.035). Spearman correlation analysis showed that there was a positive correlation between oral Streptococcus mutans gene polymorphism and caries sensitivity (r=0.258, P=0.034).

Conclusion

The distribution of Streptococcus mutans in children′s oral cavity in the Bozhou area was different between the high caries group and the caries-free group, but there was no difference among nationalities. Streptococcus mutans in the high caries group had more genotypes than those in the caries-free group. The genetic polymorphism of Streptococcus mutans might be related to the caries-causing ability of Streptococcus mutans.

Apical sealability in extracted teeth by the root canal filling agents GuttaFlow and AH Plus: a systematic review

Objective

To compare the apical sealing effects of two root canal fillers, GuttaFlow and AH Plus, for clinical reference.

Methods

The Cochrane system evaluation method was used to search the Cochrane Library, Embase, CBM, PubMed, CNKI, Weipu, and Wanfang databases. Additionally, relevant journals and conference papers were manually retrieved, and relevant randomized controlled trials were collected. Two reviewers independently evaluated the quality of each study and extracted the data. A meta-analysis was performed using the RevMan5.3 software for homogenous studies, and a descriptive analysis was performed for studies with poor homogeneity.

Results

In total, 10 randomized controlled trials containing 398 isolated teeth were included. The meta-analysis results showed that the difference in apical microleakage was statistically significant at 1 week and 3 months [1 week: MD=-0.13, 95% CI (-0.22,-0.04), P=0.007; 3 months: MD=-1.27, 95% CI (-1.94,-0.60), P=0.0002] but not at 6 months [MD=-0.10, 95% CI (-0.26, 0.06), P=0.23].

Conclusion

Based on existing research results, GuttaFlow may achieve better results than AH Plus in the short term (≤ 1 week). Because it is subject to limitations of time, quality, and research methods, this conclusion requires more long-term, high-quality, large-sample, multimeasurement randomized controlled trials for further validation.

Research progress on the correlation between interleukin - 18 and chronic periodontitis

Chronic periodontitis is an infectious disease caused by plaque as the initiating factor. Clinically, it manifests as irreversible loss of hard tissue, leading to the destruction of surrounding periodontal tissue, including the deep periodontal pocket, loss of attachment, and finally, tooth loss. Interleukin-18 (IL-18) can promote inflammation and regulate immune function and plays an important role in mediating the host immune response and inflammatory response. An increase in IL-18 in vivo can induce the production of interferon and inflammatory factors, such as interleukin, tumor necrosis factor and matrix metalloproteinase, thus mediating the dual reaction of immunity and inflammation. These inflammatory factors are involved in the occurrence and development of chronic periodontitis. Many clinical studies have shown that the levels of IL-18 in serum, saliva, gingival crevicular fluid and gingival tissue samples of patients with chronic periodontitis may be positively correlated with the severity of periodontitis; however, as a candidate gene, IL-18 is involved in the susceptibility polymorphism of periodontitis. Understanding how to quantify the level of IL-18 in clinical studies and apply it to diagnostic tools and new sites identified by new methods (genome-wide association studies and omics research) will also deepen our understanding of the pathogenesis of IL-18 in chronic periodontitis and provide new ideas for future precision medicine and the formulation of personalized programs. In this paper, the structure, biological function and association between IL-18 and periodontitis are reviewed.

Exploration of medication rules and mechanisms of traditional Chinese medicine in the treatment of periodontal disease based on data mining, network pharmacology, and molecular docking

Objective

To explore the medication law and mechanism of traditional Chinese medicine compounds in the treatment of periodontal disease through data mining, network pharmacology, and molecular docking.

Methods

First, data mining was used to search single medicinal materials for the treatment of periodontal disease, and the active components and their action targets were screened. Second, the disease target database was employed to download the targets related to the pathogenesis of periodontal disease, map them with the action targets of traditional Chinese medicine, and obtain the targets that are considered potential targets of traditional Chinese medicine in the treatment of periodontal disease. Potential targets were analyzed for gene ontology function and signaling pathway. They were then screened to obtain the key targets for the treatment of periodontal disease. Finally, the active components were docked with key targets.

Results

Among the traditional Chinese medicine prescriptions for the treatment of periodontal disease, Shudihuang, Mudanpi, Danggui, Fuling, Jinyinhua, Shanyao and Zhimu had the highest frequencies. Forty-three active components and 118 action targets were screened, and 52 potential targets were obtained by intersection with 856 disease targets. The molecular functions and biological processes in which potential targets may participate mainly focus on vitamin D biosynthesis and RNA polymerase Ⅱ regulation and involve 96 signaling pathways. Through the analysis of network topology parameters, 11 key targets were obtained. The results of molecular docking showed that the active components and RAC-alpha serine/threonine-protein kinase (AKT1), cellular tumor antigen p53 (TP53), and mitogen-activated protein kinase-1 (MAPK-1) have good binding activity.

Conclusion

Traditional Chinese medicine compounds may play a role in the treatment of periodontal disease by inhibiting alveolar bone absorption, have antibacterial and anti-inflammatory properties, and promote tissue repair. The effective treatment of periodontal disease by traditional Chinese medicine compounds provides a more scientific reference to the sustainable development of traditional Chinese medicine.

Principal component analysis of dental caries related factors among 3- to 5-year-old children in Urumqi City

Objective

To explore dental caries-related factors among children aged 3-5 years in Urumqi City and to provide evidence for the etiological study and primary prevention of caries among local children.

Methods

A multistage stratified cluster sampling method was used to randomly select children aged 3-5 years in Urumqi City for oral examination, physical examination and questionnaire survey. Data were collected and analyzed by principal component logistic regression using SPSS 23.0 software.

Results

Principal component analysis showed that there were seven risk factors whose characteristic root value was greater than 1: oral hygiene habits, family education level, sugar intake, bedtime eating habits, age, gestational age, feeding mode, and cumulative contribution were 66.486% of the total variance. Principal component logistic regression analysis indicated that five factors, namely, oral hygiene habits (OR = 0.795, P =0.044), family education level (OR = 0.667, P = 0.019), sugar intake (OR = 1.260, P = 0.006), bedtime eating habits (OR = 5.432, P<0.001) and age (OR = 0.676, P = 0.015), were closely related to early childhood caries, and they were statistically significant (P<0.05).

Conclusion

According to the principal component analysis, oral hygiene habits factor, family education level factor, sugar intake factor, bedtime eating habits factor and age factor were dental caries related factors among 3- to 5-year-old children in Urumqi City.

Arginine-containing desensitization toothpaste for dentin hypersensitivity: a Meta analysis

Objective

Meta analysis was used to evaluate and compare the efficacy of treatment of dentin hypersensitivity with arginine desensitization toothpaste and commonly used desensitization toothpaste.

Methods

A computerized search of Wanfang, PubMed, web of science, and Cochrane library databases was performed, and a manual retrieval was supplemented. Randomized control trials (RCT) about arginine-containing desensitization toothpaste for dentine hypersensitivity were collected, then data extraction and quality assessment were performed by 2 raters independently. Meta-analysis was carried out by RevMan 5.3 software.

Results

A total of 7 RCTs were included. All the results were evaluated by the tactile sensitivity test and the air blast test. The mean and standard deviation of each treatment group were analyzed by the random effects model. Meta-analysis results showed there was statistical significance in the therapeutic effects between arginine-containing desensitization toothpaste and potassium-containing desensitization toothpaste(tactile sensitivity test to evaluate: SMD = 1.83, 95% CI BP, 2.58], P<0.05; cold air assessment: SMD = -1.19, 95% CI [-1.69, -0.69], P<0.05); However, there was no statistical significance between arginine-containing desensitization toothpaste and strontium-containing desensitization toothpaste (tactile sensitivity test to evaluate: SMD = 0.68, 95% CI [-1.59, 2.96], P>0.05; cold air assessment: SMD = -0.94, 95% CI [-2.28, 0.40], P>0.05).

Conclusion

It shows that arginine-containing desensitization toothpaste has greater effects than potassium-containing desensitization toothpaste for dentin hypersensitivity, but no better effects than strontium-containing desensitization toothpaste.

A case-control study on the association of Hashimoto’s thyroiditis and anti-thyroid antibodies with oral lichen planus

Objective

This study aims to explore the association between oral lichen planus (OLP) and Hashimoto’s thyroiditis (HT) and its anti-thyroid antibodies to provide clinical evidence for thyroid disease screening in patients with OLP.

Methods

This study was approved by the institutional ethics committee. A total of 125 clinically and histopathologically confirmed patients with OLP were enrolled as the case group, and they were matched with 125 non-OLP controls based on sex and age. Demographic data (gender, age, lesion type, and disease duration) were collected from both groups. Serum levels of thyroid peroxidase antibodies (TPOAb) and thyroglobulin antibodies (TgAb) were measured to analyze their associations with sex, age, lesion type, and disease duration in patients with OLP.

Result

The prevalence of HT in patients with OLP was 31.20%, significantly higher than that in the control group (9.60%) (χ2=18.504, P<0.001). The prevalence of HT in female patients with OLP (39.13%) was significantly higher than that in male patients (9.09%)(χ2=10.93,P<0.001). The positivity rate of thyroid peroxidase antibodies (TPOAb) in patients with OLP (17.6%) was significantly higher than in the control group (4.0%) (χ2=10.989, P<0.001). The TPOAb positivity rate was significantly higher in female patients (22.83%) than in male patients (3.03%) (χ2=5.210, P=0.014). There was no statistically significant difference in the positivity rate of TgAb between patients with OLP (7.2%) and the control group (3.2%) (P>0.05). Patients with erosive lesions had a significantly higher TPOAb positivity rate (25.0%, 17/68) compared to those with non-erosive lesions (8.77%, 5/57), and the difference was statistically significant (χ2=4.831, P=0.028). Logistic regression analysis revealed that female patients with OLP had an 8.935-fold higher risk of being TPOAb positive compared to males (OR=8.935, 95%CI: 1.134-70.388, P=0.038). Patients with erosive OLP lesions had a 3.199-fold higher risk of TPOAb positivity compared to those with non-erosive lesions (OR=3.199, 95%CI: 1.064-9.618, P=0.038).

Conclusion

The prevalence of HT is higher in patients with OLP, with higher positivity rates of anti-thyroid antibodies observed in female patients and those with erosive OLP lesions. This suggests that thyroid disease screening should be incorporated into the clinical management of patients with OLP, especially for women and patients who present with erosive lesions.

Caries prevention mechanism of the Xinjiang Mori Cortex based on network pharmacology and its effect on the main cariogenic bacteria

Objective

To investigate the potential caries prevention mechanism of the Xinjiang Mori cortex and to analyze its effect on the main cariogenic bacteria.

Methods

The active components of the Xinjiang Mori cortex and the main targets were predicted and screened using the TCMSP database. The GeneCards, DisGENET and TTD databases were used to obtain caries-related targets. The common targets were derived, and core genes were screened. The enrichment analysis was performed using the DAVID data platform. Molecular docking was performed using AutoDock software. In in vitro antibacterial experiments, first, the 50% minimum inhibitory concentration (MIC50) and the minimum bactericidal concentration (MBC) of the Xinjiang Mori Cortex extract against Streptococcus mutans, Streptococcus sanguis and Actinomyces viscosus were determined and the growth curves were measured. The effects of the Xinjiang Mori Cortex extract on acid production, polysaccharide production and adhesion ability of Streptococcus mutans, Streptococcus sanguis and Actinomyces viscosus in the planktonic state were determined. The 50% minimum biofilm inhibition concentration (MBIC50) and 50% minimum biofilm reduction concentration (MBRC50) were determined by crystal violet staining, and biofilm morphology was visualized using scanning electron microscopy (SEM).

Results

The main active components of the Xinjiang Mori cortex included quercetin, kaempferol, and β-sitosterol. Tumor necrosis factor (TNF), interleukin-6 (IL-6), and interleukin-1beta (IL-1β) could be the most important targets of the Xinjiang Mori cortex for the prevention of dental caries. The enrichment analysis results showed that Mori cortex extract may have effects on the AGE-RAGE signaling pathway, IL-17 signaling pathway, and TNF signaling pathway. The antibacterial experiment results showed that the MIC50 values of Xinjiang Mori Cortex extract against Streptococcus mutans, Streptococcus sanguis and Actinomyces viscosus were 0.5, 0.5 and 0.25 mg/mL, respectively, and the MBCs were 4.0, 2.0 and 1.0 mg/mL, respectively. The inhibitory effect of Xinjiang Mori Cortex extract on the acid production, polysaccharide production and adhesion ability of three major cariogenic bacteria in the planktonic state was stronger than that of the control group, and the differences were statistically significant (P<0.05). The MBIC50 was 1.0, 1.0, and 0.5 mg/mL, and the MBRC50 was 4.0, 4.0, and 2.0 mg/mL. SEM observation showed that the amount of biofilm formation decreased with the drug concentration compared with the control group.

Conclusions

Xinjiang Mori cortex extract can prevent caries through quercetin, kaempferol, and β-sitosterol active ingredients, TNF、IL-6、IL-1β key targets and multiple pathways and inhibit the growth, acid production, polysaccharide production, and adhesion ability of three major cariogenic bacteria in the planktonic state and has some inhibitory effect on corticogenic biofilm formation.
