Metabolic hypertension is a prevalent chronic disease characterized by elevated blood pressure and is closely associated with energy metabolism disorders. Current Western medicine treatments have limitations in improving metabolic disturbances, while conventional traditional Chinese medicine pattern differentiation fails to adequately reveal the intrinsic pathological connections. Based on clinical practice, classical theories, and modern medical research, we propose that the dynamic imbalance of the Yangming-Jueyin system constitutes the core pathogenesis: beginning with Yangming stagnation due to dietary irregularities, which then leads to "Earth stagnation Wood depression" (deficiency arising from excess) and finally develops into "Wood depression Earth stagnation" and "wind-yang disturbing the upper" (excess arising from deficiency). These two processes interact as both cause and effect, ultimately resulting in a complex condition characterized by qi consumption and yin damage, deficiency below and excess above, with turbid stasis obstructing the collaterals. This paper systematically elucidates this dynamic pathological evolution and proposes the core therapeutic principle of clearing Yangming and regulating Jueyin. Specifically, clearing Yangming directly addresses the stagnation of metabolic turbid pathogens to purify the source of transformation, whereas regulating Jueyin harmonizes the contradictory state of inhibited dispersion and hyperactive wind-yang, thereby settling wind-yang and restoring the function of dispersion. This diagnostic and therapeutic approach aims to overcome the limitations of previous static pattern classifications and establish a dynamic, holistic framework, providing novel theoretical and practical foundations for the traditional Chinese medicine diagnosis and treatment of metabolic hypertension.
