Abstract

Infertility caused by Zhengjia (pelvic mass) represents an explanatory and classificatory extension of infertility due to pelvic factors within the framework of traditional Chinese medicine. It is characterized by uterine masses and reproductive dysfunction, with complex etiology and challenging treatment, and current therapeutic approaches remain limited. This study establishes a "regulating rhythm and restoring roundness" diagnostic and therapeutic system for infertility associated with pelvic masses, grounded in the theory of circular motion and integrated with the modern biological clock system. The theory of circular motion and the biological clock system are highly consistent in their emphasis on cyclical processes, dynamic balance, and hierarchical regulation, jointly elucidating the spatiotemporal unity of life rhythms. Disruption of the biological clock impairs the coordination of daily, monthly, and annual rhythms, leading to a pathological progression from "macro-rhythm disorder" to "meso-organ imbalance" and finally to "micro-level formation of pelvic mass." Macro-level disruption of the daily rhythm causes yin-yang imbalance; monthly rhythm disturbance leads to qi and blood disharmony; at the annual level, impaired synchronization results in improper storage and discharge. At the meso-level, dysfunction of the liver, spleen, and kidney contributes to qi and blood stagnation, internal dampness and phlegm, and essence and blood deficiency. At the micro-level, phlegm and stasis block the uterine meridians, ultimately forming a pelvic mass that hinders conception. Treatment adheres to the principle of "regulating rhythm and restoring roundness, " establishing a three-dimensional strategy of "macro-rhythm regulation, " "meso-organ adjustment, " and "micro-level mass elimination." Timed medication harmonizes the day-night and yin-yang fluctuations; cycle regulation restores the uterine storage and discharge; seasonal health practices align with the growth, harvest, storage, and rest. By promoting liver function, strengthening the spleen to resolve dampness, and replenishing kidney essence, the "circular communication" of the zang-fu organs is restored. When combined with syndrome differentiation and individualized treatment, this approach reconstructs the reproductive microenvironment. The proposed system transcends the limitations of solely treating the masses, offering novel insights into improving pelvic mass infertility.
