Highlights
• Mitochondrial unfolded protein response (UPRmt) acts as a central mitochondrial stress-integration hub in trauma, ischemia–reperfusion injury, and critical illness.
• Dysregulated UPRmt drives sterile inflammation, immune dysfunction, and the progression to systemic inflammatory response syndrome and multiple organ dysfunction syndrome.
• UPRmt plays a context-dependent role in aging by maintaining mitochondrial proteostasis and metabolic homeostasis, thereby influencing longevity and age-related disease susceptibility.
• Precise and context-dependent modulation of UPRmt represents a promising therapeutic strategy for trauma and organ dysfunction syndrome.
