Highlights
• Chronic diabetic foot ulcers (DFUs) are driven by persistent inflammation, impaired vascularization, and microbial burden that standard care fails to resolve.
• Silk fibroin (SF) scaffolds (i.e. electrospun mats, freeze-dried constructs, and hydrogels) enable tunable, controlled delivery of bio-additives to dysregulated wound environments.
• Bio-enhanced SF scaffolds accelerate wound closure, modulate inflammatory and angiogenic pathways, and promote tissue regeneration across preclinical diabetic wound models.
• Clinical translation is limited by regulatory complexity, manufacturing scalability, and the lack of clinically relevant wound models; smart and responsive SF systems represent a priority direction for future work.
