• The ontogeny and polarization dynamics of macrophages underscore their indispensable roles as master regulators of repair processes, and macrophages orchestrate the wound healing in different phases.
• In diabetic wounds, macrophages exhibit progenitor stage maturation anomalies, recruitment and transmigration impairments, maturation and phenotypic imbalance, substantial deficits in tissue repair function, immunometabolic disruption, and epigenetic rewiring.
