Abstract

Creatine monohydrate supplementation is best known as an ergogenic aid. However, its potential role in recovery from muscle disuse atrophy has attracted increasing scrutiny in healthy and clinical populations. This narrative mini-review examines human studies across immobilization, reloading, orthopedic and medical rehabilitation, and exercise-induced muscle damage, with close attention to supplementation timing relative to the recovery phase. During brief periods of immobilization, creatine can increase intramuscular creatine availability, but controlled lower-limb studies show that it does not consistently preserve muscle cross-sectional area or strength. Once progressive loading resumes, findings are more favorable. Creatine has accelerated recovery of quadriceps cross-sectional area and knee extensor power during resistance-based rehabilitation after immobilization, and individual trials report added benefit when paired with strengthening in knee osteoarthritis, Parkinson's disease, and inflammatory myopathies. In contrast, evidence is null or mixed for total knee arthroplasty, early inpatient stroke care, chronic obstructive pulmonary disease (COPD), and peripheral arterial disease (PAD). Studies of exercise-induced muscle damage provide a parallel recovery model, but trial-level findings and meta-analyses remain inconsistent. Current evidence does not support creatine as a stand-alone strategy during strict unloading. Its more plausible role is as a low-risk adjunct during reloading, when progressive resistance exercise and adequate nutrition restore an anabolic environment. Future studies should separate immobilization from active rehabilitation, quantify rehabilitation dose, verify creatine uptake when possible, and prioritize clinically meaningful functional outcomes.
